PTTD is a progressive condition where the posterior tibial tendon (PTT), which normally supports the medial longitudinal arch of the foot, becomes injured—through overuse, degeneration, or sometimes acute rupture. Over time, the tendon’s ability to support the arch is compromised, leading to the collapse of the medial longitudinal arch, hindfoot (heel) valgus, forefoot abduction, and often pain and dysfunction of the foot/ankle.

The posterior tibial muscle is located in the back part of the lower leg. It travels behind the inner ankle bone, makes a noticeable turn, and attaches to several areas in the foot, including a spot on the navicular bone and the bottom surfaces of other foot bones. Because of its path and the way it moves, this muscle can experience stress and strain, especially at the point where it bends.
Repetitive strain leads to degeneration of tendon fibers, potential tendinopathy, tendon elongation or partial tearing.
As PTT weakens, the spring ligament and other medial soft tissues compensate; this increases stress andleads to arch collapse.
Pain and tenderness is palpable along the posterior tendon.
It often presents as an inability to balance well while walking on uneven surfaces as well as not being able to tip toe properly without support.
