About Fungal Nail Treatment

At DC Podiatry, we offer PDT, a modern, non-invasive treatment for fungal nail infections (onychomycosis). This therapy uses a combination of photosensitizing medical gel and a specific wavelength of red light (630 nanometer) to selectively destroy fungal cells while preserving the surrounding healthy nail tissue.

How The Treatment Works

Toluidine blue gel application

  • A specially formulated photosensitive blue gel containing toluidine blue O applied to the affected nail.
  • Fungal cells absorb the dye more readily than healthy nail tissue.
  • This makes the fungal structures more sensitive to light activation.

Activation with 630nm red light

The nail is then exposed to the red light at 630 nanometers, a wavelength that:

  • Penetrates the nail plate effectively
  • Activates the photosensitiser within the fungal cells
  • Triggers a photochemical reaction that produces reactive oxygen species

Selective fungal cell destruction

These reactive oxygen molecules break down fungal cell membranes and internal structures, leading to fungal destruction, while leaving the healthy nail and skin unaffected.

Sessions Needed

  • During the first 1-2 weeks, 3-4 sessions are needed. Two follow-up sessions will be scheduled—one at the one-month mark and another at the two-month mark.
  • By the third-month assessment, we can determine whether the treatment has worked. If it has, new healthy nail growth should be visible at the base of the nail, as this is the amount of time needed for new nail to start growing out.
  • After successful treatment, prophylaxis treatment every 4-6 months is advised to ensure that fungus does not regrow. There are no side effects from prolonged use of this treatment as it only targets fungal cells with the blue gel application.
